Diabetes Self-management Education

Basics Class Learn to manage diabetes, stay healthy and prevent complications from diabetes. Begins with an individual assessment with a registered nurse, who becomes your case manager. A registered dietitian also will be assigned to you. Both are certified diabetes educators. Next is an all-day class team-taught by certified diabetes educators (RNs and registered dietitians), a board-certified endocrinologist, a physical therapist and an exercise physiologist. Participants can attend a follow-up breakfast seminar each quarter for the remainder of the year. Available at Piedmont Hospital. Physician referral required.

Intensive Diabetes Management Class For people who know the basics of diabetes management but need updated or advanced skills in self-management. Recommended for those on multiple daily injections or an insulin pump, or for those considering an insulin pump. Covers carbohydrate counting, dosage adjustment, pattern management and review of acute diabetes complications. Physician referral required.

Diabetes Consultation One-on-one outpatient consultations by appointment for topics that are not appropriate to a group setting and for people requiring individual care. Available at Piedmont Hospital and Piedmont Fayette Hospital. Physician referral required.

Basic Diabetes Meal Planning A great review for those who have not had diabetes education recently and need more current, in-depth nutrition education. Physician referral required.

Carbohydrate Counting A two-hour class on managing diabetes through carbohydrate counting. Physician referral required.

Gestational Diabetes For expectant mothers diagnosed with gestational diabetes. Covers individualized meal planning, exercise and blood-glucose monitoring for a healthy pregnancy. Available at Piedmont Hospital and Piedmont Fayette Hospital. Physician referral required.

Diabetes Prevention Program A 12-week course for people at risk for developing diabetes. Teaches preventive techniques and skills through nutrition, exercise and behavior modification. Team-taught by a registered dietitian, an exercise physiologist and a registered nurse. Physician referral required.

Grocery Store Tour Conducted by a registered dietitian who is also a certified diabetes educator. Learn to select foods and read and interpret labels for greater flexibility in healthy meal planning. To register, call 1-866-900-4321.

Foot Care Low-cost, basic foot care and screening are offered to Diabetes Resource Center participants unable to care for their own feet. Provided on an individual basis by a registered nurse/certified diabetes educator. Referrals from your resource center case manager and an appointment are required.

Insulin Pump Education For people starting on insulin pump therapy or upgrading to the latest pump. Follow-up provided as needed. Physician referral required.

Diabetes/Insulin Pump Support Group 404-605-3823 Meetings include pump users, physicians, certified diabetes educators and manufacturers’ representatives. Meets once every three months in the Richard H. Rich Auditorium. Call for dates.

Diabetes Support Group 404-605-3823 Meets third Mondays from 7:30 p.m. to 9 p.m. in Classroom 5, 77 Building.
