The physicians, nurses and specialists on staff at Piedmont Hospital's Diabetes Resource Center strive to provide the quality education and care that you need for your diabetes. The care you receive here fulfills Medicare's Medical Nutrition Therapy (MNT) Benefit. If you are enrolled in Medicare, your Diabetes Self-Management Training will be covered by your insurance. You are entitled to 10 hours of education your first year, thereafter you may receive the MNT Benefit as listed below.

To be eligible for the MNT Benefit, you must meet the following requirements:

• Diabetes diagnosis
• Benefit consists of a maximum of three hours in the initial episode of care (calendar year) and two hours each subsequent year with a physician's referral.
• You must receive care from a registered dietitian who is enrolled as a Medicare provider.
• You must be referred to the program by your physician; non-physician practitioners cannot refer.

Unfortunately, you are not eligible for the MNT benefit if you are receiving maintenance dialysis. To ensure that you are covered, please indicate MNT on your regular Diabetes Resource Center referral form.
